Who should use this form

This form is intended for Queensland Health staff coordinating a local Maternity Emergency Program (MEP) delivery in a regional, rural or remote maternity facility. MEP Foundation and MEP Advanced are recognised as meeting the mandatory maternity emergency training requirements for midwives and medical officers working within Queensland Health maternity services and are accredited by RANZCOG and ACM for continuing professional development (CPD). Use this request form if your service is planning an MEP Foundation course, an MEP Advanced course, or both, and you are seeking onsite course delivery support..
  • This form is

    • A centralised intake for sites to provide local details, availability and preferred site visit dates for 2027.
    • A way to help CSDS RRR team evaluate and schedule RRR site visits.
    • Open to QLD Health staff only (Sign-in required)

  • This form is not

    • An automatic booking confirmation.
    • A way to hold dates before CSDS staff availability has been confirmed.
    • For any years other than 2027. The team require 90 days notice for support visits, so we want to aim for only 2027 dates.

What to have ready before you start

Having this information ready will make the form faster to complete and reduce back-and-forth after submission.
  • Your service details

    • Your name, role and contact details.
    • Your Hospital and Health Service and facility or site.
    • The course/s you want to run during the CSDS site visit: Foundation, Advanced, or both.

  • Date preferences

    • Up to 3 proposed dates in preference order for both MEP-FC and MEP-ADV
    • Confirmation you will be able to fill 6-12 participant spots for dates selected.
    • Whether the course requires full delivery by the MEP RRR team or if a co-facilitation model will be used.

  • Faculty information for co-facilitation

    • The faculty support your site can provide.
    • The names and email addresses of local co-facilitators available for your preferred dates.
    • Please note: all local co-facilitators need to be accredited MEP faculty. Find out information about the MEP faculty pathway.

What happens after you submit

After submission, your request moves into the QME RRR team workflow for review, planning and scheduling.

  1. Your request is received — The QME RRR team receives your site details, course type, faculty information and preferred dates.
  2. The request is reviewed — The team reviews your request against program requirements, local faculty capacity, scheduling demand and available support.
  3. Scheduling is coordinated — If the request can progress, the QME RRR team works through scheduling and coordination using the information you supplied.
  4. You are contacted — The QME team will contact you with the outcome and next steps. Requests are supported where possible, but submission does not guarantee support.
